1. Recognize and understand that martial arts training is an activity that involves physical contact and that my participation might result in serious injury, including permanent disability or even death, and severe social and economic loss.

2. Recognize and understand that such risk may be due to not only my own actions, but also the action, inaction or negligence of others, the regulations of participation, or the conditions of the premises or of any of the equipment used.

3. Recognise that there may be other risks that are not known to me or to the others or not reasonably foreseeable at this time.
4. Agree to inspect the facilities, equipment and pairings prior to participation. I will immediately inform an instructor if I believe that anything is unsafe or beyond my capability and refuse to participate.
5. Assume all of the foregoing risks and accept personal responsibility for any damages that may result from injury, permanent disability or death.

6. Enter martial arts training and/or competition entirely of my own free will and understand the importance of following the rules of training and competition.

7. I certify that I am in good physical condition, and have no disease, injury or other condition that would impair my performance or physical and mental well-being during intense training practice and/ or competition.
8. Grant permission in case of injury to have a doctor, nurse, athletic training or other emergency medical personnel provide me with medical assistance or treatment for such injury.
9. Release, waive, discharge and covenant not to sue, Grapple Zone LDN LTD Martial Arts, its affiliated organisations and governing bodies, their officers, instructors and personnel, other members of the organizations, participants, supervisors, coaches, sponsoring organizations or their agents, and if applicable, owners and leasers of the premises from any and all liability to the undersigned, his or her heirs and next of kin for any and all claims, demands, losses and damages
which may be sustained and suffered on account of injury, including death or damage to property, caused or alleged to be caused in whole or in part by the negligence of the releases or otherwise.

I HAVE READ THE ABOVE WARNING, WAIVER, RELEASE, AND ASSUMPTION OF RISK. I FULLY UNDERSTAND, AND THAT I HAVE GIVEN UP SUBSTANTIAL RIGHTS BY SIGNING IT. I HEREBY SIGN IT VOLUNTARILY WITHOUT ANY INDUCEMENT, ASSURANCE, OR GUARANTEE BEING MADE TO ME AND INTENDED MY SIGNATURE TO BE A COMPLETE AND UNCONDITIONAL RELEASE OF ALL LIABILITY.

PHOTO RELEASE FORM

I hereby grant permission to Grapple Zone LDN Ltd and its agents or employees to photograph me and/or my minor child/children during classes, events, or activities associated with Grapple Zone LDN Ltd. I authorize the use of these photographs for inclusion in publications such as brochures, newsletters, magazines, display boards, and electronic media, including websites, social media platforms, and digital publications. I further permit Grapple Zone LDN Ltd to share these photographs for use or distribution in non-company publications, in print or electronic form, without requiring further notification to me.

I waive any right to inspect or approve the finished photographs or any printed or electronic materials in which they may appear, whether such use is known to me or unknown at the time of signing. I also waive any right to royalties or other compensation arising from or related to the use of these photographs.

I agree to release, defend, and hold harmless Grapple Zone LDN Ltd and its agents or employees, as well as any third-party publishers or distributors involved in producing or disseminating the final materials, from any claims, damages, or liability arising from or related to the use of the photographs. This includes, but is not limited to, any alleged misuse, distortion, blurring, alteration, optical illusion, or composite use—intentional or unintentional—that may occur during the creation, processing, publication, or distribution of the images.

DOJO RULES & CODE OF CONDUCT

● Always book your class and arrive early. If you are late, do not step onto the mats without the instructor’s permission.
● You must be checked in to attend any class.
● Do not train if you are sick, feeling unwell, or have any skin infection.
● Do not step off the mats without the instructor’s permission.
● Leave all footwear on the shoe rack by the entrance.
● Never walk barefoot into the toilet area — wear your own footwear or use the sandals provided.
● Wash your hands after using the bathroom.
● Do not change in open areas; use the changing rooms.
● Keep fingernails and toenails short and clean.
● Always shake hands or bump fists before and after sparring.
● Apply submissions with care and control.
● Rude or inappropriate language will not be tolerated.
● Any form of discrimination will result in immediate cancellation of membership.
● No food is allowed inside the dojo.
● Keep water bottles off the mats.
● No clothing with zippers, buckles or metal parts is allowed on the mats.
● Clean your feet with antibacterial spray before stepping onto the mats.
● When others are sparring, please sit on the bench; if space is limited, step off the mats to maximise room.
● If a partner taps, release the submission immediately.
● Practise only what the instructor is teaching unless told otherwise.
● Do not talk while the instructor is teaching.
● Recording on mobile phones is not allowed without permission.
● If you sustain an injury, notify the instructor immediately and complete an incident report.
● Parents: please ensure your child uses the bathroom before class.
● No jewellery or piercings may be worn during training.
● Remove makeup before training to avoid staining uniforms.
● Treat training partners, instructors, and teammates with respect.
● Control your temper — bullying or physical outbursts will not be tolerated. If you appear to have lost control, you will be asked to leave the mats.
● Help keep the dojo clean, tidy, and safe at all times.
● Maintain good personal hygiene and avoid bad body odour.
● Respect other people’s values and beliefs.
● A clean gi and rashguard must be worn for BJJ sessions. For No-Gi, a rashguard, shorts, and/or spats are required.
● All cuts and abrasions must be properly covered before training. If you bleed on the mats, notify the instructor and clean the area with the antibacterial wipes provided.

All forms of discrimination against any Stakeholder are considered serious misconduct. Complaints will be taken extremely seriously and dealt with appropriately.

• Maintaining a training environment free from bullying, harassment, victimisation, and unlawful discrimination — one that promotes dignity, respect, and the recognition of individual differences.

• Providing training to managers and staff on their rights and responsibilities under this policy, including their duty to support equal opportunities and prevent discriminatory behaviour.

• Offering opportunities for training, development and progression to all coaches and staff. We aim to support everyone in achieving their full potential so that individual talent can be utilised to maximise organisational effectiveness.

All decisions concerning coaches and staff will be based on merit, except where lawful exemptions permitted under the Equality Act apply.

Complaints Procedure

Any Stakeholder who believes they have been treated unfairly under this policy has the right to raise a complaint through the appropriate procedure. All concerns will be fully investigated, and appropriate disciplinary action will be taken where discriminatory conduct is found.

Coaches and staff will be subject to Grapple Zone LDN Ltd’s internal investigatory and disciplinary procedures. Individuals raising a genuine grievance will not be penalised for doing so, unless a claim is found to be knowingly false or made in bad faith.

MEMBERSHIP POLICY

Membership Cancellation

• Membership at Grapple Zone LDN Ltd runs on a monthly basis and operates as a rolling contract with a minimum term of one month.

• Members may cancel their membership after, or with effect from, the end of the minimum term by giving one full calendar month’s written notice, effective from the date the notice is received. Cancellation requests must be sent by email to admin@grapplezone.co.uk.

• A Grapple Zone LDN Ltd team member will respond to all cancellation requests. If confirmation is not received, it is the member’s responsibility to re-submit the request.

• It is the member’s responsibility to instruct their bank to cancel the Direct Debit after the final payment has been made. Grapple Zone LDN Ltd is not obliged to refund fees where the member has failed to cancel the instruction and cannot provide proof of a valid cancellation request. All fees and charges must be fully settled before the Direct Debit is cancelled.

Re-joining Fee

• Any member who cancels their membership and later wishes to re-join will be subject to the current joining fee and will be placed on the current membership tariff listed at:

Membership Freeze

• Members may freeze their membership after, or with effect from, the end of the minimum term by giving one full calendar month’s written notice, effective from the date the notice is received. Freeze requests must be emailed to admin@grapplezone.co.uk.

• A Grapple Zone LDN Ltd team member will confirm all freeze requests. If confirmation is not received, the member is responsible for re-submitting the request.

• It is the member’s responsibility to instruct their bank to cancel the Direct Debit after the final payment has been made. Grapple Zone LDN Ltd is not obliged to refund fees where the member has not cancelled the instruction and cannot provide proof of a valid freeze request. All outstanding fees must be cleared before the Direct Debit is cancelled.
