WARNING, WAIVER, RELEASE OF LIABILITY, ASSUMPTION OF RISK
THIS AGREEMENT MUST BE SIGNED BY ALL PERSONS WHO WISH TO PARTICIPATE IN ANY GRAPPLE ZONE LDN LTD MARTIAL ARTS, EVENT, OR ACTIVITY.

1. Recognize and understand that martial arts training is an activity that involves physical contact and that my participation might result in serious injury, including permanent disability or even death, and severe social and economic loss.
2. Recognize and understand that such risk may be due to not only my own actions, but also the action, inaction or negligence of others, the regulations of participation, or the conditions of the premises or of any of the equipment used.
3. Recognise that there may be other risks that are not known to me or to the others or not reasonably foreseeable at this time.
4. Agree to inspect the facilities, equipment and pairings prior to participation. I will immediately inform an instructor if I believe that anything is unsafe or beyond my capability and refuse to participate.
5. Assume all of the foregoing risks and accept personal responsibility for any damages that may result from injury, permanent disability or death.
6. Enter martial arts training and/or competition entirely of my own free will and understand the importance of following the rules of training and competition.
7. I certify that I am in good physical condition, and have no disease, injury or other condition that would impair my performance or physical and mental well-being during intense training practice and/ or competition.
8. Grant permission in case of injury to have a doctor, nurse, athletic training or other emergency medical personnel provide me with medical assistance or treatment for such injury.
9. Release, waive, discharge and covenant not to sue, Grapple Zone LDN LTD Martial Arts, its affiliated organisations and governing bodies, their officers, instructors and personnel, other members of the organizations, participants, supervisors, coaches, sponsoring organizations or their agents, and if applicable, owners and leasers of the premises from any and all liability to the undersigned, his or her heirs and next of kin for any and all claims, demands, losses and damages
which may be sustained and suffered on account of injury, including death or damage to property, caused or alleged to be caused in whole or in part by the negligence of the releases or otherwise.
I HAVE READ THE ABOVE WARNING, WAIVER, RELEASE, AND ASSUMPTION OF RISK. I FULLY UNDERSTAND, AND THAT I HAVE GIVEN UP SUBSTANTIAL RIGHTS BY SIGNING IT. I HEREBY SIGN IT VOLUNTARILY WITHOUT ANY INDUCEMENT, ASSURANCE, OR GUARANTEE BEING MADE TO ME AND INTENDED MY SIGNATURE TO BE A COMPLETE AND UNCONDITIONAL RELEASE OF ALL LIABILITY.

PHOTO RELEASE FORM

I grant permission to GRAPPLE ZONE LDN LTD and its agents or employees, to use photographs taken of me or my minor child/ children in classes or events of GRAPPLE ZONE LDN LTD for use in publications such as brochures, newsletters, and magazines, and use the photographs on display boards, and to use such photographs in electronic versions of the same publications or on GRAPPLE ZONE LDN LTD websites or other electronic forms of media, and to offer them for use and distribution in other non company publications, electronic or otherwise, without notifying me.
I hereby waive any right to inspect or approve the finished photographs or printed or electronic matter that may be used in conjunction with them now or in the future, weather that use is known to me or unknown, and I waive any right to royalties or other compensation arising from or related to the use of the photograph.
I hereby agree to release, defend, and hold harmless GRAPPLE ZONE LDN LTD and its agents or employees, including any firm publishing and/or distributing the finished product in whole or in part, whether on paper or via electronic media, from and against any claims, damages or liability arising from or related to the use of the photographs, including but not limited to any misuse, distortion, blurring, alteration, optical illusion or use in composite form, either intentionally or otherwise, that may occur or be produced in taking, processing, reduction or production of the finished product, its publication or distribution.

DOJO RULES & CODE OF CONDUCT

● Always book your class and, and arrive early, if you are late do not step onto the matted area without the permission of the instructor.
● Without a booking you cannot attend classes
● Do not train when you are sick or feeling sick or have a skin infection of any sort
● Do not step off the mat without permission of the instructor
● Leave footwear on the shoe rack by the door
● Do not go barefooted into the toilet area, collect your footwear and return it after use or use the sandals provided
● Wash your hands after going to the bathroom
● Do not change in the open, use the changing rooms
● Ensure Fingernails and toenails are cut short
● Always shake hands or bump fists before and after sparring
● Apply all submissions with extreme care and control
● Rude language will not be tolerated
● Discrimination of any sort will not be tolerated and those responsible will have their membership cancelled
● No food products to be brought into the Dojo
● Water bottles kept off the mat
● No zippers or buckles on clothing used for training
● Rub your feet with antibacterial before you step on the mat
● While others are sparring, please sit on the bench and if not enough space step off the edge of the matted area to maximize space.
● If someone taps let go of the submission immediately
● Practice what the instructor is teaching (unless instructed to do any other technique)
● Do not talk while the instructor is teaching
● No recording on mobile phones without permission
● If you sustain an injury, please notify the instructor as soon as possible and fill in an incident report
● Parents – ensure your child has visited the bathroom prior to class
● No jewelry or piercings should be worn during training
● Remove any makeup as this can stain your opponents uniform
● Treat your training partners, instructors and team mates with respect
● Control your temper (bullies and physical outbursts will not be tolerated). If, at any time, you look to have ‘lost control’ you will be asked to leave the training mat. It is everyone’s responsibility to ensure the Dojo is clean, tidy and safe at all times.
● Participants must be clean and void of all bad body odor
● Everyone should be respectful of other people's values and beliefs
● Clean gi and rashguard to be worn for BJJ sessions and Rashguard, shorts and spats must be worn for any non gi session
● All cuts and abrasions must be covered adequately before commencing training If you bleed on the mat, please notify the instructor and clean up the blood with antibacterial wipes provided

• Provide equality, fairness and respect for all in our community.
• Provide a space where all stakeholders are welcome, comfortable and free of any discrimination.
• Eliminate any discriminant of Stakeholders under the Equality Act 2010 protected characteristics of age, disability, gender reassignment, marriage and civil partnership, pregnancy and maternity, race (including colour, nationality, and ethnic or national origin), religion or belief, sex and sexual orientation.
• Eliminate any discrimination based on socio-economic circumstance and build an environment where everyone no matter their background feels equal, safe and welcome.
• Oppose and avoid all forms of unlawful discrimination.
At Grapple Zone LDN LTD we commit to:
• Encourage equality, diversity and inclusion in our space as they are fundamental to our core beliefs as a community space.
• Eliminate all forms of discrimination. This entails ‘direct discrimination’ – treating someone less favourable than you would treat others due to a protected characteristic. ‘Indirect discrimination’ – if there is a practise that applies in the same way but disadvantages those with a protected characteristic. ‘Bullying’ – misusing one’s power to humiliate or undermine the confidence of another individual. ‘Victimization’ – displaying detriment to someone who has taken action in good faith under any principles outlined in the 2010 Equality Act. ‘Harassment’ – violating the personal space and engaging in unwanted conduct whether it is based on a protected characteristic or if it is an advance of a sexual nature. Sexual harassment may amount to a criminal matter, such as in sexual assault allegations. In addition, harassment under the Protection from Harassment Act 1997 – which is not limited to circumstances where harassment relates to a protected characteristic – is a criminal
offense.
At Grapple Zone LDN LTD all forms of discrimination against a Stakeholder are to be classed as serious misconduct. Complaints will be handled with the utmost importance and action will be taken appropriately.
• We will maintain a training environment free of bullying, harassment, victimisation and unlawful discrimination, promoting dignity and respect for all, and where individual differences and the contributions of all Stakeholders are recognised and valued. This commitment includes training managers and all other staff about their rights and responsibilities under the equality, diversity and inclusion policy. Responsibilities include staff conducting themselves to help the organisation provide equal opportunities, prevent bullying, harassment, victimisation and unlawful discrimination.
• Make opportunities for training, development and progress available to all coaches and staff, who will be helped and encouraged to develop their full potential, so their talents and resources can be fully utilized to maximize the efficiency of the organisation.
All decisions concerning coaches and staff are based on merit (apart from any necessary and limited exemptions and exceptions allowed under the Equality Act).

Complaints procedure.
To safeguard individual rights and adhere to our Grapple Zone LDN LTD Policy any Stakeholder who believes they have been wrongfully treated under anything in this policy may raise the matter through the correct procedure. Matters will be fully investigated and correct disciplinary procedures will be taken against any ‘stakeholder’ who wrongfully discriminates. Coaches and staff will be subject to our internal investigatory and disciplinary procedures. Any individual raising a grievance will not be penalized for doing so unless it is untrue and not made in good faith.

MEMBERSHIP POLICY

Membership Cancellation
* Membership at Grapple Zone LDN LTD runs on a monthly basis and is a rolling contract with a minimum term of one month.
* Members may cancel their membership after or with effect from the end of the minimum term, by giving an advance one full calendar month written notice, effective from the 1st of the following month. This should be done via sending an email to admin@grapplezone.co.uk.
* A Grapple Zone LDN LTD team member will respond to all cancellation requests. If confirmation of cancellation is not received, the member is responsible for re-submitting the cancellation request.
* It is the responsibility of the member to advise their bank to cancel the Direct Debit instruction after the final payment has been made. Grapple Zone LDN LTD is not obliged to refund any fees where the member has not canceled the instruction and cannot provide proof of a valid cancellation request. All fees/charges must be cleared prior to canceling the direct debit.
Re-joining Fee
* Any member who cancels their membership and wishes to re-join will be subject to the current joining fee and will be placed on the current membership tariff as stated on https://www.grapplezone.co.uk/membership
Freeze Membership
* There is no membership freezing policy. All memberships are on a rolling contract; the membership must be terminated and resignation will be required. Any joining fees will apply.
